Your $100 Booking Deposit / Doctor Consult includes:

Hormones are never one size fits all. Some women only need testosterone. Some do better with estrogen and/or progesterone. Others need a combination. That’s why we call it Hormone OPTIMIZATION.
You’ll only EVER pay one of two amounts:
- 1. $100 · Your plan is pharmacy meds, no testosterone. The deposit covers all 4 things above & you owe us nothing more. You only pay the pharmacy for your meds.
- 2. $299 total · Your plan includes testosterone. Your $100 deposit counts toward it & $199 is due after your consult. Gets you all 4 things above PLUS your testosterone, supplies, & FREE shipping to your door in Arizona.

- 1. We call in a lab order to Sonora Quest, & you use your own insurance to pay for the lab draw.
- 2. Self-pay & we draw labs at our office (~$140 initial draw, ~$120 follow-ups).
- 3. If you have hormone labs within 3 months, we might be able to use those.

Do I need new labs for my refill?
Yes. Every consult needs current labs, so plan on new bloodwork at least every 3 months. Your provider has to see the numbers to go through them with you at the consult. Labs from another clinic or another draw work if they are less than 3 months old & have what the provider needs. Email them to us first & we will tell you if they work.
Do I have to do another consult to refill?
Yes. Your refill includes a 20-min consult with one of our providers, online or in person. You go over your labs together & build your game plan.
What does my refill cost?
You’ll only ever pay one of two amounts. $100 if your plan is pharmacy meds with no testosterone, & you only pay the pharmacy for those meds. $299 total for 3 months if your plan includes testosterone, & your $100 booking deposit counts toward it, so $199 is due after your consult. Bloodwork is required & is not included in the price.
Will my estrogen or progesterone get shipped to me?
No. Only testosterone ships. Your testosterone & all supplies ship free to your door, anywhere in Arizona. Estrogen & progesterone are called in to your local pharmacy, or picked up at our Mesa office.
How early should I start my refill?
Start as early as you can. Nothing is kept in stock at the 503A compounding pharmacy, so it is about 10 days from the pharmacy request to you, & your meds are not called in until after your consult.
Is my body scan included?
Yes. Your body scan is included, & scans are Mon-Thurs 10am to 5:30pm. Just tell us when you are coming.
It has been a while since my last consult. Can I still refill?
If it has been more than 8 months since your last consult, fill out the new patient intake form. It asks a few extra questions we need after that long away. You still get returning patient pricing, not new patient pricing. If your last consult was more than a year ago, you also need a new consult before we can send more meds.
Combat Medic Wellness prescribes testosterone only for the treatment of clinically diagnosed testosterone deficiency. We do not prescribe testosterone for bodybuilding, athletic performance, or aesthetic purposes, and requests for such use are declined.
