If you have not started hormone therapy with us yet, you are a new patient. If you have already been prescribed HRT through us, use the returning patient button to request your refill.
If it has been more than 8 months since your last consult, fill out the new patient intake form. It asks a few extra questions we need after that long away. You still get returning patient pricing, not new patient pricing. If your last consult was more than a year ago, you also need a new consult before we can send more meds.
Booking takes a $100 deposit. It is not an extra fee. It goes toward the price of your meds.
Your provider builds your plan from your consult & your lab results. You can get labs drawn at our office, ~$140 for the first draw & ~$120 for follow-ups, use your insurance at Sonora Quest, or bring hormone labs from the last 3 months.
No. Hormone treatment is Arizona only. Our Utah telehealth covers weight loss & mental health.
Testosterone ships free to your door in Arizona. Estrogen & progesterone never ship. Those are called in to your local pharmacy, or you pick them up at our Mesa clinic.
