Fill out a 5-minute Intake form to get started.

Before you can meet with the Provider for your consultation, you will need your hormone levels tested. 

(Unless you already have hormone level lab results within the last 3 months)

If you don't have a lab draw company you prefer, contact us and our Doctor will call in an order to Rupa Labs

You will then schedule with Rupa labs to have them come out to your home or office for the blood draw.

Once the results are back, you can move onto step four which is booking your online consult with Dr. Manchak

After the Dr. consult, 3 months of meds will be mailed to your house with all supplies you need to get started
