CANCELLATION/NO-SHOW/RESCHEDULE/REFUND POLICY
Online Doctor Consults:
Patients must notify CMW by text message (not voicemail) at 480-401-2312 at least 24 hours in advance to cancel or reschedule an online consult appointment.
Patients who miss their scheduled online consult or are unprepared at the time of the appointment, for any reason, will incur a $49 rebooking fee.
If a patient declines to pay the $49 rebooking fee and instead requests a refund, the patient will be refunded all amounts paid minus $49 for the consult time that was booked and not attended.
Patients acknowledge and agree to this fee because missed or unprepared appointments result in lost revenue, as CMW is not provided sufficient time to schedule a replacement patient for the missed consult slot.
Approximately 24 hours prior to the scheduled online consult, patients will receive a text message at the phone number provided on the medical history form outlining exactly what is required to be considered “prepared” for the consult. Failure to follow these requirements will result in the $49 fee being withheld from any refund.
If the phone number provided on the medical history form is inaccurate and the provider is unable to contact the patient for the scheduled consult, a $49 rebooking fee will be required to proceed.
Medications Returns/Refunds
Medications that have been picked up by the patient and have left the office are not eligible for return or refund. Medication vials that are broken or left unrefrigerated while in the patient’s possession are not eligible for a refund.
Medications shipped directly to the patient from a compounding pharmacy are guaranteed by the compounding pharmacy to arrive in satisfactory condition. If a medication arrives in an unsatisfactory condition, the patient agrees to contact the compounding pharmacy directly to request a replacement.
If a patient believes a vial was underfilled, the patient agrees to contact the compounding pharmacy directly. Any determination regarding additional medication is solely between the patient and the compounding pharmacy.
