Online Consults:

Patients shall notify CMW by text (not voicemail) at 480-304-0249 a minimum of 24 hours in advance that they want to cancel/reschedule their online consult appointment.

Patients who miss or are unprepared for their scheduled online consult (FOR ANY REASON) will incur a $49.00 rebooking fee.

If the patient declines to pay the $49 rebooking fee & instead chooses to receive a refund, the patient will be refunded all the money they paid, MINUS $49 for the consult time they booked but did not attend.

Patients agree to incur this fee because they understand that if they miss their appointment or are not prepared for their appointment (FOR ANY REASON), CMW incurs lost revenue because they were not provided adequate time to find a replacement patient for the missed consult time.

Prior to your online consult, patients will be texted (to the phone number they provided on the medical history form) the details of EXACTLY what must be done to be considered "prepared" for your online consult. Failing to follow the pre-set guidelines will result in the $49 fee being withheld from your refund

If the number you provided in your medical history form is not accurate and as a result, the doctor cannot contact you for your scheduled consult - a $49 rebooking fee be required to move forward


Medications:

Medications that have been picked up by the patient and left the office cannot be returned for a refund Medication vials that break wall in the patient's possession cannot be refunded

Medications that are shipped directly to the patient via a compounding pharmacy are guaranteed (by the compounding pharmacy) to arrive in a satisfactory condition. If the medication's arrive in an unsatisfactory condition, the patient agrees to contact the compounding pharmacy directly for a replacement.
