Terms and Conditions
- Services must be paid for at the time of service.
- Health insurance typically does not cover services provided at Combat Medic Wellness. If you want to seek insurance reimbursement, we would be happy to provide you itemized invoices that you can submit to your insurance company.
- If you are late or miss your appointment, you may be subject to a $50 fee.
- You must agree to take medications as prescribed and follow the medical provider’s instructions. Selling or sharing your prescriptions with other individuals is prohibited.
- Treatments used at Combat Medic Wellness might not be considered a medical necessity. Treatments rendered are for the purpose of improving quality of life through hormone restoration, nutritional and supplemental counseling, and weight loss treatment.
- Patients are provided with our medical director’s phone number at time of consult. If you are experiencing any side effects or become sick, we strongly recommend that you: contact our medical director, follow-up with your primary care provider, or go to an urgent care or emergency department as soon as possible.
- Combat Medic Wellness will not be your primary care provider unless you elect them so. If not elected, you must agree to continue to follow-up with your primary care provider and notify them of treatments prescribed at Combat Medic Wellness.
- Making an appointment with Combat Medic Wellness does not necessarily entitle you to being issued a prescription for hormone replacement, weight loss medication, or additional medications/supplements. Every individual is different, and it is at the medical provider’s discretion to issue a prescription.
- You must maintain your follow-up appointments and submit regular updated medical health history forms to remain on treatment. It is important that lab work is monitored regularly for safety purposes. It is important that Combat Medic Wellness’ medical provider manages your treatment, and it is at their discretion to provide treatment and/or medications.
- You will be advised of the risks and benefits of treatment and possible complications and side effects both verbally and in writing.
- We do not provide any age-related preventive care on a routine basis, and you will be required to follow-up with your primary care provider to obtain these screenings, and you may not hold Combat Medic Wellness and/or their providers responsible if an adverse event occurs during your treatment. You must ensure that your primary care provider provides the results of such screenings to Combat Medic Wellness as this could change the treatment prescribed to you.
Return and Refund Policy

- There are no refunds for services or products rendered. We cannot accept back used medications once they have been dispensed per state regulation. Consults are non-refundable once completed. Prior to the time of your consult (up to 12 hours in advance), you may cancel your appointment and request a refund.
- If you are deemed ineligible by our licensed provider in pre-screening for any of our products, a full refund will be provided for any deposits or purchases made. You may contact us directly at 480-934-0829 or via email to [email protected] to seek refund or check on status.
